MEDICALS

ALL medicals are to be returned to Judy Wall: Not to coaches, Not to other athletes, Not EVEN to Lester.  Please make sure every line is completed, the form is legible, and is signed by the doctor.  Mail or deliver them to Judy.  If your athlete is scheduled for a physical, call Judy to see if their medical is going to expire soon and she will mail you a new form to be completed.  If the doctor has any changes to medication or restrictions for your athlete, please ask for the changes in writing and mail a copy to Judy.  If you have a change of address or phone number, Judy will also need to know this information. 
Athletes without a current medical will not be able to participate in any Special Olympics event or function.  You will be called to come and pick them up.  Thank you for your cooperation.
